Community-Based Adult Services in San Diego and Surrounding Areas
Assessments
Our multidisciplinary team members conduct a comprehensive evaluation of each new participant and recommend support services based on their specific needs, goals and desired outcomes.
Nursing Services
Our professional team of nurses coordinates care, manages medications, observes and monitors participants’ health status and any changes in their condition. The individual medical, cognitive or mental health condition and risk factors are taken into consideration.
Personal Care Assistance
Our caregivers provide continuous care under nursing supervision to ensure participants’ safety and protection. Assistance is provided with walking, grooming, dressing, eating and all personal hygiene needs including toileting and continence care. Cleanliness and safety are essential to our quality of care.

Eligibility for CBAS
Individuals may be eligible for CBAS if they meet one or more of the following criteria:
- Individuals who meet Nursing Facility Level of care.
- Individuals who have an Organic, Acquired or Traumatic Brain Injury and/or Chronic Mental Illness.
- Individuals with Moderate to Severe forms of Alzheimer’s Disease or other Dementia.
- Individuals with Mild Cognitive Impairment including Moderate Alzheimer’s Disease or other Dementia.
- Individuals with Intellectual Disabilities and Meet the Criteria for San Diego Regional Center Eligibility.
CBAS Objectives
- Rehabilitate and/or maintain participant’s level of care to the maximum extent possible.
- Maintain the participant in the community for as long as possible.
- Prevent inappropriate or premature institutionalization.
- Assist participant’s family/caregivers by providing respite care to their loved ones.

Families Deserve A Break
Caring for a loved one with a debilitating disease such as Alzheimer’s Disease, Stroke or Parkinson’s Disease, requiring constant attention and supervision is exhausting!
Using Horizon CBAS to supplement the care you provide at home allows the family / caregiver respite time.
Horizon CBAS also allow their loved ones to remain at home as long as medically, socially and economically feasible.
Blood Pressure can be monitored daily when your loved one is at the center.
